Module Aims

This module aims to enhance healthcare professionals knowledge and understanding of Long Term Health Conditions and its management to enable Service Users to live with the conditions and improve their health and wellbeing.

Content Summary

Students should be aware they may encounter ethical issues in learning and teaching while undertaking this module and should refer to either the Award/Module Leader for further information.
Strategies to manage Long Term Health Conditions e.g.: Expert patient programmes, integrated care. Bio-psychosocial and environmental factors which influence the conditions and affect health and well-being. Appropriate theories including organisational management, whole systems, change management, partnership and collaboration. Ethics in care, quality evaluation and outcomes measurement. National and International evidence and policy that influences practice. The evidence base for care and enhancing health and well-being across the lifespan.
