Module Aims

To understand ways of improving the health and wellbeing outcomes of individuals with a range of needs or are at risk, using communication and planning strategies.

Apply psychological theory to support the development of interventions to address biological, psychological and social (biopsychosocial) determinants affecting health and wellbeing

To explore how theories of health improvement are applied practically, e.g. Make Every Contact Count (MEC) and Motivational Interviewing.

Content Summary

Contemporary Government legislation (e.g. Social Services and Well-being (Wales) Act and Well-being of Future Generations (Wales) Act 2015).

Principles underpinning effective communication and mechanisms to promote person centred outcomes, e.g. person centred reviews, inclusive assessments of need and person centred care and support plans.

Biological interventions (e.g. medication and pharmacology interventions for addictions).

Psychological interventions (e.g. cognitive behavioural therapy), social humanistic approaches to therapy, active listening and emotional intelligence

Sociological interventions (e.g. tackling social exclusion and health inequalities as well as promoting personal and social resilience) to contribute towards positive wellbeing outcomes for individuals at risk.

These interventions will be explored within the context of individuals at risk who need care and support such as children and young people, older people, individuals with substance and alcohol misuse issues and people who need care and support.

Creating supportive environments and settings to improve community health and wellbeing.

Communicating health messages including brief interventions.

# Learning Outcome
LO1 Identify the interdependence between the biological, psychological and sociological (bio-psycho-social model) aspects of people’s lives to adopt the most appropriate intervention in consultation with an individual at risk.
LO2 Apply the principles of communication to the development of a Care and Support plan with an individual to engender rapport and trust to create an honestly informed plan to improve wellbeing outcomes.
