Module Aims

The module aims to facilitate the development of the practitioner’s knowledge, skills and decision making in the assessment and care planning

processes.

Content Summary

Developing advanced skills relating to the assessment and planning process, utilising a range of data and observational sources supported by appropriate technology including assessment diagnostic tools.

Analysing complex assessment information and data in the planning immediate and ongoing care needs.

Assessing across a range of contexts and situations – acute, long term, transitional and end of life assessment of care needs.

Assessing transition of care needs in partnership with people who use services, their carers and other professionals and service providers.

Risk assessment in challenging and unpredictable situations.

Exploration of new and emerging science and technology to inform assessment and care planning processes

Using clinical reasoning, problem solving, influencing and negotiation skills in decision making and care planning.

Developing advanced communication and relationship management strategies in the assessment and care planning processes.

Effective partnership working and shared decision making in the co production of plans of care.

Applying principles of capacity and informed consent through assessment and planning processes, making best interest decisions as required.

Making reasonable adjustments to help people who use services, and their carers understand their assessment and identify in partnership their care needs.

Effectively supporting those who are vulnerable during the assessment process, enacting safeguarding protocols if required.

Underpin assessment and planning process to consider individual preferences, their home and support systems, culture and beliefs, ensuring equality, diversity and inclusion.

Facilitate maximum independence and self-care, acknowledging the individuals’ preferences as to where care is delivered.
