Module Aims

The module aims to further develop knowledge and skills to deliver a range of interventions in the provision and evaluation of care based on best practice

Content Summary

Developing knowledge to manage and evaluate complex episodes of care throughout the duration of the care period.

Understanding and applying a range of evidence-based interventions including complex care, therapeutic interventions, social prescribing, acknowledging individuals’ preferences, beliefs, and cultural requirements.

Managing complex medicines administration, optimisation, and medicines reconciliation, evaluating the impact and effectiveness of the medication.

Managing transition of care in partnership with people who use services, their carers and other professionals and service providers.

Recognising reduced concordance, changes in motivation or dissatisfaction with the care and treatment plan. Working in partnership with people to revise plans of care as required.

Effectively evaluate in partnership plans of care and treatment options and adjust as required.

Maintaining effective therapeutic relationships with people, their families and/or carers throughout the episode of care and treatment, acting as an advocate and support person as required.

Supporting people who use services, children and young people and families through and beyond diagnosis.

Using a range of techniques to educate people who use services and their carers to promote independence and confidence in supported self-care and self-management.

Decision making, and risk management in challenging and unpredictable situations.

Working with people in the provision of evidence-based person-centred nursing care for those who are dying or near to the end of life.

Maintaining clear records in writing or digitally relating to decisions, actions taken and resulting outcomes.
