Module Aims

The module aims to further extend the knowledge, understanding of interventions aimed at maximising health protection and the promotion of health and wellbeing for people, families, communities, and populations related to their field of practice.

This module will enable the community nurse to develop specialist skills to work effectively with people who use services and their carers to make positive choices to improve their physical, mental, and behavioural health.

Content Summary

Analysis and application of epidemiology, genomics and epigenetics demography and social determinants of health and illness in the delivery of community care, locally and nationally.

Understanding influences leading to inequalities in health outcomes.

Exploring policies and initiatives locally and nationally which impact on health and well-being.

Analysis and application of Welsh Government (2018) A Healthier Wales.

Assessing and profiling health needs and support services people, communities, and populations.

Supporting positive health choices and actions.

Undertaking health screening and community profiling, identifying needs, opportunities, and services available to support health and well-being and improve health outcomes.

Critical appraisal and application of models of health promotion, concepts of motivation and empowerment and motivational interviewing.

Social prescribing to support health and well-being.

Helping people, families, and communities to engage effectively with networks and services that support health and well\\u0002being.

Active development and dissemination of inclusive information relating to communicable diseases, infection prevention and control and immunization.

Establishing therapeutic relationships through use of effective interpersonal skills to support people’s decision making and choices relating to their health and well-being.
